There are three forms that we ask prospective families to complete to begin the application process. They are available to be downloaded here:
Application (please attach a photo of your child for identification purposes) with application fee ($100): application_1011.doc
Teacher Recommendation Form (to be given to your child’s current teacher and returned to CCS directly by them): kinder-first_teacher_evalutaion.doc or second_and_up_teacher_evalutaion.doc
Release of Information Form and Permission for a CCS Staff Member to Observe (to be given to your child’s current school): parent_authorization.doc
