There are three forms that we ask prospective families to complete to begin the application process. They are available here:
Application Fill out our online application form or download the application (CCS_Application.doc) and return it to the CCS office. Please remember to attach a photo of your child (for identification purposes) and the application fee of $125.
Teacher Recommendation Form (to be given to your child’s current teacher and returned to CCS directly by them): kinder-first_teacher_evalutaion.doc or second_and_up_teacher_evalutaion.doc
Release of Information Form and Permission for a CCS Staff Member to Observe (to be given to your child’s current school): parent_authorization.doc
